Attachments:

1. Safety certificate in Arabic issued by an approved civil defense laboratory.
2. A copy of agency registration in the ministry of finance.
3. Passport photocopy of the owner.
4. A valid copy of the chamber of commerce's license including specifying civil defense activity along with maintenance contract + subscription in smart services.
5. Agency contract attested by UAE embassy in the country where the contract was signed or by the notary public in case of local contracts in Arabic.
6. Final approval certificates for safety confirmation.

Fee: AED1000 for each system + 300 Dh card fee for single system engineer or technician+50 Dh for every additional system added on the engineer or technician card+ AED1000 for the 1st equipment + 100 Dh for each additional equipment+ 10 Dh 'Knowledge Fee' for each Request + 10 Dh 'Knowledge Fee' for each card.

• Dept.of Economic Development form or letter from the company to obtain license for chemicals and HAZMAT approving and trading.

• Partnership contract in case of partners or more than one owner.

• Undertaking letter for compliance with the preventive requirements if the location is not ready for operation.

ج 103: يجب عدم إستخدام نظام حقن الهواء من مصدر أو نقطة واحدةsingle point injection system إذا كان بئر السلمstairwell يمتد إلى أكثر من ثمان طوابق. و يجب توفير نظام حقن الهواء من مصادر متعددة multiple point injection system في بئر السلم الممتد الى أكثر من ثلاث طوابق فوق سطح الأرض او يبلغ ارتفاعه إلى 15 متر، وأي مبنى أقل من ذلك في الارتفاع.

IV.    الشهادات العلمية للمهندسين مصدقة من سفارة دولة الإمارات ووزارة الخارجية في بلد المنشأ
V.    نسخة من الإتفاقية مع بيت الخبرة العالمي مصدقة من سفارة دولة الإمارات العربية المتحدة ووزارة الخارجية في بلد المنشأ
VI.    نسخة عن رخصة غرفة التجارة والصناعة سارية المفعول مع إضافة النشاط الخاص بالإدارة العامة للدفاع المدني
VII.    شهادات الخبرة للمهندسين مصدقة من سفارة دولة الإمارات ووزارة الخارجية في بلد المنشأ

الرسوم: 1000 درهم عن كل نظام + 300 درهم بطاقة للمهندس او الفني مع نظام واحد + 50 درهم لكل نظام إضافي على بطاقة الفني او المهندس + 10 درهم رسوم المعرفة على الطلب

7. Engineers' Experience certificates attested by the UAE embassy and the Ministry of Foreign Affairs on the country of origin.
