1. One passport size photograph.
2. Passport photocopy with a valid visa/ visit visa.
3. Qualification certificates attested by the UAE embassy or the Ministry of Foreign Affairs.
4. Experience certificates attested by the UAE embassy or the Ministry of Foreign Affairs.
5. Copy of the company license approved as an installation & maintenance company.
6. Copy of the approved technician card.
7. One personal photograph of the technician.

Fee: technician approval Request on each system = 100Dh + 10 Dh 'Knowledge Fee'

Attachments:

• Letter from the company including all information related to HAZMAT, its size, class and quantity.
• Trade or industrial license of the importing company.
• Copy of the registration card of HAZMAT transporting vehicle.
• Copy of the Air Way bill.

Fees: AED500 + 10 Dh 'Knowledge Fee'.

• Passport size photograph
• Approving signature from ministry of labour or Establishment card or authorization from the court
• An official letter to DCD requesting for the (issuance / renewal of the representative stamped with official stamp and signed by the authorized person).

If the requesting person is an engineer(university certificates or engineer card must be provided to prove his status as an engineer or helper)

• trade license with partnership attachment
• if the requesting person's visa is on another company for the same partners, the trade license of that company and partnership attachments must be provided.

Fee: AED500 + 10 Dh 'Knowledge Fee'.
